Certificate of Orgin
POLICY – CERTIFICATES OF ORIGIN
All Certificates of Origin must be completed and notarized prior to presentation to the Fredericksburg Regional Chamber of Commerce (FRCC). Note: The Chamber will only review forms for goods originating within the United States. We do not sign for Certificates of Origin for goods originating outside the United States.
We will stamp the Certificate of Origin by appointment only. Appointments are available Monday – Friday from 9:30 AM – 4:00 PM. Call (540) 373- 9400 to schedule an appointment. Note: Because only the Chamber president can sign Certificates, do not wait until the day of your shipment to schedule an appointment.
An Affidavit of Origin and Indemnification Agreement, printed on company letterhead, must be signed by a company officer and presented to the Fredericksburg Regional Chamber of Commerce for our records. For FRCC members, this document can be signed yearly and will be kept on file. For non-FRCC members, an Affidavit of Origin and Indemnification agreement must be provided to the Chamber each time a Certificate of Origin will be issued. This document must be signed and notarized before the Certificate of Origin is issued.
A Certificate of Origin Disclaimer, printed on company letterhead, must accompany each Certificate of Origin.
For FRCC members in good standing, there is no fee for each certificate stamped. For Non-FRCC members, there will be a fee of $125 per certificate stamped. Payment is due at the time of service and can be made by cash/check or credit card, payable to FRCC.
The following guidelines must be adhered to:
- The proper Certificates of Origin form must be used for the country to which you are exporting.
- The Certificate must be filled out completely and accurately.
- Product information listed is verbatim to what is on the invoice. Do not leave off product number, quantities, or descriptions.
- The Certificate may not be altered in any way.
- The Certificate must be notarized prior to presentation to the Chamber.
The Chamber has the right of refusal for any document that does not meet FRCC Policy requirements.
